TheCase Management Specialistfunctions as a member of the discharge planning team assigned to coordinate the care and services for assigned patients for an episode of illness or treatment in collaboration with the patient, family, physician, patient care team, and payers. The Case Management Specialist utilizes communication, organizational, and problem-solving skills to implement the post-hospital care plans and interventions defined by the RN Discharge Coordinator. The Case Management Specialist promotes and evaluates the effective utilization of resources using current knowledge and awareness of community resources. The position assumes a pivotal role with the patient care team to achieve optimal clinical and resource outcomes.
In this role, excellent communication skills, telephone etiquette and interpersonal skills are crucial. Required qualifications include the ability towork independently, organize, and prioritize work with attention to detail; and the ability to collaborate as a team member and communicate effectively with other departments, physician offices, patients, families, community providers, and staff.
Education: An Associate degree or equivalent in education and/or additional related experience is required (2 additional years of experience if no AA degree). Associate or Bachelor s degree in health care or related field ispreferred.
Experience: At least3 years experience coordinating care in an acute care, skilled nursing, or home health setting with demonstrated ability in effective communication and administrative functions is required. Also required is experience coordinating with community resources; demonstrated knowledge in applying specific criteria for accessing those resources; and experience with Microsoft Word and Excel.
Preferredexperience includes advanced experience using Microsoft Word and Excel and a current understanding of CMS regulations related to hospital discharge requirements.
